 Second PCDC Learning Collaborative Held

The second CCBC Learning Collaborative Session was held Wednesday, May 9, 2012 and Thursday, May 10, 2012. Five primary care coaches from Primary Care Development Corporation (PCDC) facilitated this second learning session for 16 CCBC clinic partner Change Teams and Leaders. The focus of the session was Population Management, Risk Stratification and Registries.

This CCBC Learning Session created a community learning environment and encouraged creative thinking and goal setting around how to operationalize Registries and Risk Stratification into the chronic care service delivery at participating organizations. One of the 78 participants noted that “the other clinics were very open minded about sharing and networking information with everyone” while another noted that they found the “explanation and activity around developing a registry” to be useful.

Electronic Medical Record vendors for the various change team organizations participated in this Learning Session by providing a demonstration of how Registry and Risk Stratification functionality worked in their system.

The goal is to educate and empower the teams on definitions and management of a population of patients while incorporating evidence-based guidelines into the structure of how they provide care.  PCDC incorporated feedback from Learning Session #1 by making this session educational as well as entertaining by including a skit on how to handle change team dynamics. A participant noted that “the meeting was hands on, very informative and fun.”

This Learning Session initiates the second intensive activity period.  Change teams will return to their centers and continue to meet weekly or bi-weekly with their coaches to operationalize the goals they set forth during their work plan development at the start of the project.
