Inaugural CCBC Learning Collaborative Held

The first of five CCBC Learning Collaborative Sessions was held Tuesday, March 13, 2012 and Wednesday, March 14, 2012. Five primary care coaches from Primary Care Development Corporation (PCDC) facilitated this inaugural learning session for 18 CCBC clinic partner’s Change Teams and Leaders. The focus of the session was quality improvement, principles of care management and change management.
This first CCBC Learning Session created an environment designed to encourage creative thinking and goal setting around improving the efficiency and performance level of chronic care service delivery at participating organizations. One of the 90 participants noted that the learning session helped them understand that “vast information and help is out there”, while another noted that “the entire day was a learning process!”
PCDC creates structured content-rich presentations for each Learning Session. The goal is to educate and empower the teams on definitions and standards related to the particular subject matter of Quality Improvement and Principles of Care Management. The didactic portions of the day were coupled with dynamic, interactive team activities that:
(1) Encourage development of new ideas or highlights existing best practices;
(2) Drive health care professionals to share experiences and knowledge with each other
Many participants voiced their appreciation for co-consulting activities, in which teams were mixed, to share ideas and challenges in a structured format. One participant stated, “I like interacting with other centers and learning about their challenges and realizing that we have the same challenges and hearing their ideas about addressing the challenge.”
To date, the change teams have participated in an intensive planning period in which they worked with their individual coaches on a weekly or bi-weekly basis to complete assessments and create workplans to address CCBC interventions. They will return to their centers and initiate an activity period to put what they have learned into practice. Their next Learning Session will be held in May 2012.
